After fifteen years, Alex Karev (Justin Chambers) has left Grey’s Anatomy. He was one of two original interns remaining on the show, the other being Meredith Grey. His departure was both unexpected and, of course, tear-jerking. Over the past fifteen seasons, we’ve seen Alex grow from a headstrong bully into an amazing friend, uncle to Meredith’s children, and pediatric surgeon.

He’s had many relationships, two wives, and (it seems like) a million one night stands. We don’t know if we’ll ever see Alex again on screen, but even if we do, he won’t be a series regular again. In light of his departure, let’s dive in and look at some things we never got to see this Iowa wrestling champion do.

10 Improve Pac-North

When this plotline was dropped, it should have been a red flag that Justin Chambers was on his way out of Grey’s Anatomy. Despite Alex Karev taking a Chief of Surgery job at Pac-North and committing to fixing the place up with a crew composed of bad Pac-North doctors, Richard Webber, and (almost) Maggie Pierce, the hospital ended up being bought by the Fox Foundation due to a dispute between Richard and Catherine. The resolution of this plotline was unrelated to Alex, and since his farewell episode only featured letters he wrote to specific people, we never even got to hear what he thought of the acquisition of Pac-North.

9 Reignite His Africa Project

In an earlier season, Alex began a project in which impoverished kids with serious illnesses would be flown to Grey Sloan and receive pro bono surgeries. When the kids arrive, the hospital is overjoyed that Alex found the money to pull off the gargantuan project.

The hysterical twist ends up being that Alex had no money at all he maxed out all of his credit cards and emptied his savings account just to get the kids to Seattle. One of those kids ends up being Zola, who Derrick and Meredith immediately begin to love. The “Africa Program” as it’s called in the show isn’t mentioned again after this season, but since it’s one of the most incredible things Alex ever did, it would have been great to hear it being mentioned or, even better, to see it being continued.

8 Reunite With Arizona Robbins

Arizona Robbins, played by Jessica Capshaw, trained Alex Karev to be a pediatric surgeon, and right before she left he was really mad at her for leaving. Ironically, Arizona also left the show to make her family whole by living in the same city as Callie so Sophia could have two moms. It’s even implied in her last scene that she and Callie got together again off-screen. Since Arizona’s ending so closely resembled Alex’s, it would have been especially nice to see her return before Alex left, perhaps to offer him some classic Arizona wisdom.

7 Reunite With Izzie (On Screen)

Since Justin Chambers left suddenly, he wasn’t actually in his farewell episode. Instead, the episode consisted of him narrating several letters he sent to his favorite Seattle residents. In the first letter, Alex tells Meredith that he reconnected with Izzie before her trial and learned that Izzie had his children, two five-year-old twins.

As Alex reads the letter, we get some very brightly lit clips of two children running around happily, jumping into bed to hug their parents, a blonde woman and a man with short brown hair. These actors were not Katherine Heigl and Justin Chambers, so their faces were never shown. The fact that Alex left Seattle to be with Izzie truly brings his plot to a comfortable close, but it was frustrating to not get to see that.

6 Raise A Kid (On Screen)

In Alex’s farewell episode fans got to see some truly cute images of his children playing, showing him toys, and just generally being adorable on Izzie’s farm. A lot of Alex’s plot involved him shedding his tough exterior and learning to let people in. Some of the first signs that Alex is becoming kinder happen when he starts taking an interest in pediatric surgery. Alex ends up becoming an incredible pediatric surgeon, and a lot of that is because of the super fun atmosphere he creates for his patients. When Meredith becomes a mother, Alex is overjoyed for her. It makes sense that Alex would leave Seattle to be a good dad to his kids, but we wish a little more of that story could have happened on screen.

5 Land A Board Seat

When Bailey won the Grey Sloan board seat, it seemed like that would be foreshadowing for some kind of redemption plotline for Alex, in which he strives to get a seat on the board. (Almost all of the other main characters have seats on the hospital board as well.)

Despite being an excellent surgeon, and later an excellent Chief of Pediatric Surgery, Alex never seems to pursue a board seat again. Since Alex is a character who comes from a lot of poverty, it would have been sweet to see him have such an acclaimed (and profitable) position in the hospital where he worked for fourteen years.

4 Wrestle

If you crave this one, you’re a longtime fan of Grey’s Anatomy. In the early seasons, Alex was a little obsessed with wrestling. He was also a little obsessed with being “Iowa Strong,” a phrase that became iconic after a gunshot victim from Alex’s hometown comes into Grey Sloan with the same fervor for wrestling as Alex. Grey’s Anatomy spent a lot of time watching Derek go fishing; it also spent a lot of time watching Christina and Meredith dance it out. It would have been cool to see Alex hit the mats and show off some skills that had nothing to do with surgery.

3 Win A Harper Avery Award

The Harper Avery award was such a driving force of action in the earlier seasons. Christina leaves to another continent after losing the Harper Avery award because she worked at a Harper Avery hospital. Meredith later wins a Harper Avery, and the moment is so defining for her that she sees her long-dead mother in the audience smiling in approval after she wins.

Though it’s repeatedly mentioned that Alex is an incredible pediatric surgeon, fans of the show never got to see Alex win any awards at all, let alone a Harper Avery. Alex never quite tackled medicine with the fervor that Christina and Meredith did, but there were a few seasons when he struggled to find a surgical specialty or project that would make him stand out. In the end, it was another plotline that wasn’t meant to be.

2 Be Happy With Jo

Fans of the Jo/Alex pairing are likely still grieving after hearing Alex’s tragic letter to Jo Karev, his wife, and longtime partner on the show. The letter, in which he called himself a coward several times, explained to Jo that he was unable to force his kids to live a fatherless life without him. Though many fans have pointed out that he could have asked Jo to go with him, Camilla Luddington was not nearly as ready to leave the show as Justin Chambers was, and so the characters were split up. Though many fans believe Alex was happier with Jo than he was with Izzie, Alex and Jo never truly enjoyed a peaceful arc in their time together. Seeing them work through their traumas and face their pasts together was beautiful, but the honeymoon definitely could have lasted longer.

1 Say Goodbye To Meredith Face To Face

Richard Webber’s famous first episode speech warns the original group of interns that not everyone would succeed—that at the end of the journey, those that remained would be few. Fifteen years later, Alex Karev and Meredith Grey were the only two original interns still on the show. Though the two initially hated each other, they bonded in their brutal honesty and twisted humor. After Christina left Seattle, Alex and Meredith became each other’s person, aka best friend. In his goodbye episode, the writers were able to highlight his Sunday breakfasts with Meredith and her family, and how close he had gotten to Meredith’s kids. Alex’s letter to Meredith was beautiful, but it’s sad she didn’t get to reply in real-time to what he had to say.
