Apple TV+’s The Shrink Next Door is a story of manipulation by eccentricity. Dr. Isaac “Ike” Herschkopf (Paul Rudd) knows how to get his way and then some. His definition of therapy is having patients make rash decisions at the expense of stability in their lives. He’s counterproductive and a swindler who looks at the Code of Ethics like an ignorable option.

Those around Herschkopf are more joyful presences, but it tends to vary based on how easily they fall for Dr. Ike’s nefarious schtick, and how long they blindly follow him instead of their own gut.

Paul Rudd may be a hilarious comedy actor, but Dr. Ike is anything but funny. He leaches off individuals under the guise of serving as their therapist. In reality, he’s utilizing various methods to steal from them.

In terms of his time with Marty Markowitz, Dr. Ike’s treachery has a substantial range. When Marty’s sister raises concerns over her brother’s treatment, Ike severs the bond between the siblings. From life-altering subtle manipulation to craftily stealing a dollar at a time, Dr. Ike consistently pushes the limits to see what he can get away with. In the end, he’d rather steal a patient’s beach house than follow the Hippocratic Oath.

6 Bonnie Herschkopf (Casey Wilson)

The underrated Casey Wilson may not have lasted very long on SNL, but she is heartbreaking in The Shrink Next Door. Unfortunately, she’s not given much to do save for look on at her husband’s abuse of Marty. With that being said, she suffers emotional abuse from him as well.

Perhaps it would have been best for Bonnie to leave her husband, but an abuser corners their victim and ensures there will be no outside interference. She gets a hint of Dr. Ike’s escalating ego and selfishness when she requests they hire a nanny. He turns down the idea swiftly, coldly, and offers no alternatives. But it’s when she sees Dr. Ike’s endless parties on Marty’s dime that the audience becomes aware of just how much she realizes. She has the courage to apologize to Marty but doesn’t say for what. Bonnie would have been more likable if she’d been direct with just one of her husband’s victims.

5 Cathy (Robin Bartlett)

Longtime AFC employee Cathy only makes intermittent appearances throughout The Shrink Next Door, but when she does show up she’s a calming presence.

It’s obvious that she cares about Markowitz even outside of the boss-employee dynamic. She worked with his parents and knew them like family, so naturally, she’s protective. At no point is Cathy unlikeable, but her delivery of the line “Yay, Marty,” when he finally severs ties with Dr. Ike takes her from likable to loveable.

4 Martin “Marty” Markowitz (Will Ferrell)

Unlike most of the characters in Will Ferrell’s best movies, Marty Markowitz is far from funny. He’s a man struggling with grief, anxiety, and any number of other debilitating issues. His sister wants him to seek help, but he has an extremely hard time making decisions.

Markowitz is so lacking in confidence that he can be hard to watch. Especially coming from Ferrell, who bursts with energy, it’s a character that seems stuck. Markowitz gains some energy in the series’ penultimate episode, but he can be a bit much up until then.

3 Bruce Dolsey (Cornell Womack)

Once Dr. Ike supplants himself into Markowitz’s work life, it naturally begins to affect his employees. The first to raise concerns is Bruce Dolsey, who never seems quite on board with Ike’s methods.

He’s the type of individual who is just trying to look out for his coworkers. He raises concerns about finances and time management to Markowitz, only to be brushed away. But he keeps trying, accurately stating that Dr. Ike is a hindrance, not an asset. Dolsey’s determination about something both he and the audience knows is correct makes Markowitz’s reaction even worse. It also makes Dolsey’s attempt even more admirable.

2 Miriam (Sarayu Blue)

TV shows like The Shrink Next Door and Dopesick display how damaging the abuse of a medical license (in its various forms) can be. Whereas that latter Hulu series showed opioid abuse on a nationwide scale, Shrink shows how one man insidiously wormed his way into the lives of many.

One of his patients, Miriam, had her life destroyed by Ike. She was instructed to sever ties with her own mother. Then, when her mother passes, Miriam holds onto a long-term pang of guilt that wouldn’t have been present had she never met her therapist. Just how addictive drugs lead to the pushing away of family, so too does association with Dr. Ike.

1 Phyllis Shapiro (Kathryn Hahn)

“That’s what a family is…a group of people you owe an apology to.” Kathryn Hahn’s Phyllis Shapiro is unfortunately only prominently featured during the miniseries’ first half. However, in that time, Shapiro makes it clear she has brains, insight, and a surplus of love for her brother.

She doesn’t even need to meet Markowitz’s new doctor to know something’s off. She had the love in her heart to suggest he see someone in a therapeutic sense. It takes a bit for him to oblige, but the look on her face when he tells her his first session went well says it all. She’s thrilled, but that quickly goes away. Regardless, Shapiro is the heart of the soul, and when Markowitz turns his back on her it’s gut-wrenching. She feels like his best chance at escaping the cycle of abuse but ends up being its first major casualty.
