Martin Scorsese isn’t the kind of director to make a psychological thriller – he usually helms tales of Catholic guilt told through the eyes of the Mafia – but when he did, he knocked it out of the park. Shutter Island stars Leonardo DiCaprio as a U.S. Marshal who arrives at a mental asylum on a remote island to investigate the disappearance of a patient.
After a series of disturbing flashbacks and plot twists, it becomes pretty clear that this case is a lot more sinister than the Marshal initially thought. So, here are 10 Twisted Thrillers To Watch If You Like Shutter Island.

15 Fight Club
Adapted from Chuck Palahniuk’s debut novel of the same name, David Fincher’s Fight Club digs its hooks into its lead character’s psychology, just like Shutter Island, and also delivers a shocking gut-punch plot twist at the end of its second act.
Edward Norton and Brad Pitt are riveting in the lead roles as the Narrator, a workaday office drone who wants to escape the mundanity of his life, and Tyler Durden, the anarchist soap maker who gets him out of his shell, respectively.
14 American Psycho
A huge part of the thrills provided by Shutter Island is that it’s unclear what’s real and what isn’t, both to the viewer and to the character at the center of it all. Based on the controversial Bret Easton Ellis novel, American Psycho does just that.
It starts off as a brutal, horror-tinged satire of the corporate lifestyle, but it grows into an involving psychological thrill-ride somewhere around the midpoint. Christian Bale is captivating in the lead role of Patrick Bateman.
13 Mulholland Drive
There’s a sense of unease throughout Shutter Island, like everything is not what it seems and all the plot threads don’t seem to fully add up and make sense.
That feeling is even more palpable throughout David Lynch’s dark Hollywood satire, Mulholland Drive, which has all the director’s signature surrealness with an added dose of bitter reality arising from his cynical take on the state of the film industry.
12 Gone Girl
Director David Fincher recruited Gillian Flynn to adapt her own novel into a screenplay when he took on the job of turning Gone Girl into a movie. Ben Affleck stars as Nick, a seemingly regular guy who becomes public enemy number one when his wife, played by Rosamund Pike, goes missing and he’s the top suspect.
There are plenty of twists along the way – and really sinister, unsettling ones at that – complemented by powerhouse performances from both Affleck and Pike.
11 Taxi Driver
Helmed by Shutter Island’s Martin Scorsese, Taxi Driver might be the darkest, most disturbing psychological thriller ever made. It tells the story of Travis Bickle, a veteran who returns from Vietnam with PTSD, gets a job as a New York cabbie to fill his sleepless nights, and decides to take the law into his own hands when he finds the city’s streets teeming with crime.
Robert De Niro gives one of his all-time finest performances in the role of Bickle, while Scorsese brings the lighting tricks and camera angles of the film noir to grimy ‘70s New York to really bring the story to life.
10 Jacob’s Ladder
Paranoid, foreboding, and surreal, Jacob’s Ladder is one of the most underrated thrillers ever made. Like Shutter Island, it takes viewers deep into its lead character’s psychology. In Jacob’s Ladder, Tim Robbins stars as Jacob, a Vietnam veteran who is haunted by his experiences from the war and tries to navigate his hallucinations to figure out what’s going on in his head.
Drawn from a script by Bruce Joel Rubin (who clearly thinks about the concept of the afterlife a lot, because it’s the focus of four of his movies: Ghost, Brainstorm, Deadly Friend, and this one), Jacob’s Ladder is a smartly made, mind-bending, beautiful psychological thriller.
9 Cape Fear (1991)
Although Shutter Island is still Martin Scorsese’s most horror-themed work to date, he has dabbled in the genre in the past. Cape Fear, Scorsese’s remake of the ‘60s classic starring Robert Mitchum, tells the terrifying tale of an escaped convict, Max Cady (Robert De Niro), targeting the family of the lawyer who put him away, Sam Bowden (Nick Nolte).
Most remakes feel unnecessary, but Scorsese wouldn’t make a movie like that for an easy paycheck. He made Cape Fear because he had his own distinctive take on the material, and he knew De Niro would play a completely different Cady than Mitchum did.
8 The Sixth Sense
M. Night Shyamalan’s breakout movie, The Sixth Sense, is also the one that established his penchant for plot twists. Haley Joel Osment stars as a troubled boy with the ability to see dead people. Bruce Willis plays the psychiatrist that the boy’s mother (Toni Collette) hires to help him with his problem.
The twist doesn’t exactly line up with the logic of the plot, but the viewer is so awestruck by the reveal that it doesn’t detract from their enjoyment of the movie. The Sixth Sense opened the door for Shyamalan’s entire career, and as of yet, he still hasn’t topped it.
7 Prisoners
Before Sicario, Arrival, and Blade Runner 2049 would make Denis Villeneuve one of the hottest directors in Hollywood, he proved his filmmaking chops with Prisoners. Hugh Jackman stars as a regular family man whose daughter and her friend are abducted while playing on the street.
As the police investigation comes up dry, and even leads to Jackman himself as a suspect, he decides to take the law into his own hands. When Jake Gyllenhaal, a quiet detective working the case, starts to follow Jackman around, the movie becomes an intense battle of wits. Prisoners succeeds by using real-world fears to prey on its audience.
6 Black Swan
Darren Aronofsky was influenced by the Dostoyevsky novella The Double when he was writing Black Swan. Having just made a movie about wrestling (The Wrestler), which Aronofsky considers to be the most lowbrow art form, the director decided to make a movie about ballet, which he considers to be the most highbrow art form.
Natalie Portman stars as a ballerina rehearsing for an appearance in Swan Lake, while Mila Kunis co-stars as her closest rival. The movie plays like cinematic poetry, with the story of Portman staging Swan Lake blending seamlessly with the actual story of the Swan Lake ballet.
5 Primal Fear
Often included on lists of movies with the most shocking plot twists, Primal Fear showed audiences a dark side to Richard Gere and birthed the career of Edward Norton. Gere stars as a defense attorney who is certain that his client, an altar boy (Norton) who is accused of murdering a high-up Catholic Archbishop, is innocent.
The movie could be categorized into a few different genres – it’s a psychological thriller, through and through, but there are also elements of the neo-noir and even the courtroom drama in here – which is always good, because it means it has a feel of its own.
4 Se7en
David Fincher directed this grisly mid-‘90s mix of film noir and slasher movie. It’s essentially a buddy cop story, with Morgan Freeman playing a veteran detective on the brink of retirement and Brad Pitt playing a hotshot young cop who is new to the unit. The two are paired together when a serial killer known only as John Doe starts targeting victims based on the Seven Deadly Sins.
Andrew Kevin Walker’s screenplay is masterfully crafted and totally unpredictable. Just when you think you know where the story is going, it takes a sharp left turn and delivers a narrative gut punch.
3 Memento
Christopher Nolan adapted the script for this dark psychological thriller from his brother Jonathan Nolan’s short story “Memento Mori.” It has a complex, layered premise involving a man with amnesia trying to figure out what happened to his wife. There are two parallel storylines, one told chronologically and one told in reverse, until they meet in the middle in the film’s final moments.
There are black-and-white scenes and color scenes to differentiate which ones are set in the past and which ones are set in the present. It sounds confusing, but when the storylines converge at the end, it all makes sense.
2 The Silence of the Lambs
This film adaptation of Thomas Harris’ crime novel of the same name was the third movie in history to win Oscars in all five major categories. Anthony Hopkins nabbed a Best Actor award with less than half an hour of screen time. Jodie Foster stars as Clarice Starling, an FBI rookie struggling to have her voice heard as a female agent in a male-dominated workplace.
She’s assigned to track down Buffalo Bill, a serial killer who has been kidnapping and skinning women. To help with her investigation, she consults the oddly charming cannibalistic murderer and psychiatrist, Dr. Hannibal Lecter (Hopkins).
1 Get Out
Jordan Peele’s directorial debut Get Out – not just a terrific movie, but a cultural landmark, too – has a similar setup to Shutter Island (and basically half of all horror movies, for that matter). Both movies begin with an unsuspecting protagonist arriving in an unfamiliar environment that isn’t all that it seems.
Whereas Shutter Island is built around the protagonist’s mental health and its effect on society, Get Out is built around the protagonist’s race. Daniel Kaluuya stars as Chris Washington, a black photographer who travels to an isolated suburban community to meet his white girlfriend’s parents. Peele’s Oscar-winning script masterfully uses racial tensions as a dramatic tool to build suspense.
