Sage is a healer that heavily prioritizes damaging foes to automatically heal in Final Fantasy XIV. They can even prevent damage from being sustained.

Sage is the fourth and brand new healer introduced for Final Fantasy XIV Endwalker. Within the new healer matrix, Sage covers the grounds of damage and preventative barriers. This gives them some overlap with White Mage and Scholar, and a complete inversion from Astrologian.
Sage offers single-target sustained healing in direct conjunction with damage spells. They can also create powerful barriers to stop damage in the first place. However, while they can deal heavy damage themselves they lack the party wide utility that comes naturally for Astrologians and Scholars. And most importantly, if the tank does suffer severe damage Sages have the hardest time restoring lost health.
Jumping into a new role all the way at level 70 will likely be extremely overwhelming. Players that have never played a healer should refrain from starting Sage right away. Instead, one should play White Mage, Scholar, or Astrologian to understand the basics. Another possibility is to purposely only do low-level sync content as Sage before heading into later dungeons and raids.
Healing Via Damage in Final Fantasy XIV
One of the key features of Sage in FFXIV is the level 4 ability Kardia. This can target one party member – including the Sage themselves – and heals them with a 170 potency cure when the Sage casts attack spells. When there are two Sages in an eight-man party, they should place their Kardias on the main tank. It can also be powered up by Soteria starting at level 35. This is not their main form of healing, but instead helps top off players’ health much like Regen for White Mage and Embrace for Scholar. The affected spells include:
- Dosis: level 1, 300 potency single-target attack.
- Phlegma: level 26, 400 potency area of effect attack, 45-second cooldown.
- Eukrasian Dosis: level 30, 40 potency damage over time, 30-second duration.
- Dyskrasia: level 46, 160 potency area of effect attack.
- Toxikon: level 66, 300 potency area of effect attack but 50% damage falloff, costs one Addersting.
- Dosis 2: level 72, 320 potency single-target attack.
- Phlegma 2: level 72, 490 potency area of effect attack, 45-second cooldown.
- Eukrasian Dosis 2: level 72, 60 potency damage over time, 30-second duration.
- Dosis 3: level 82, 330 potency single-target attack.
- Phlegma 3: level 82, 510 potency area of effect attack, 45-second cooldown.
- Eukrasian Dosis 3: level 82, 70 potency damage over time, 30-second duration.
- Dyskrasia 2: level 82, 170 potency area of effect attack.
- Toxikon 2: level 82, 330 potency area of effect attack but 50% damage falloff, costs one Addersting.
- Pneuma: level 90, 330 area of effect attack but 40% damage falloff, 600 area of effect heal, 120-second cooldown.
Buffs and Other Healing
Sage has other focused healing, utility spells, and barrier options in FFXIV. Many of these should be saved until needed, while others are worth using when their cooldown is ready. Due to many of Sage’s moves having very short range, Icarus will be very important for getting to enemies or fleeing back to safety.
- Physis: level 20, area of effect 100 potency regen, 15-second duration, 60-second cooldown.
- Holos: level 76, area of effect 300 potency regen, reduces damage taken by 10%, 20-second duration, 120-second cooldown.
- Zoe: level 56, increases the potency of the next healing spell by 50%, 30-second cooldown.
- Krasis: level 86, increases a single target’s healing by 20% from all sources, 60-second cooldown.
- Haima: level 70, grants five barriers that will each prevent up to 300 potency worth of damage, after 15 seconds all remaining barriers will expire and heal for 150 potency each.
- Panhaima: level 80 and requires completion of all Sage job quests, area of effect centered on the Sage, five barriers that prevent up to 200 potency worth of damage, after 15 seconds all remaining barriers will expire and heal for 100 potency each.
- Egeiro: level 12, resurrection
- Icarus: level 40, rush to a target up to 25 yalms away.
Resource Management in Final Fantasy XIV
Besides the standard MP, Sage must also manage two types of gauges to maximize their performance in Final Fantasy XIV. The first, Addersgall can be used to both heal and recover lost MP. The second, Addersting is gained by successfully applying barriers that wind up being fully used – expired barriers yield nothing.
Addersgall Gauge
Over time Sages will gain up to three Addersgall while in combat, making it very similar to the White Mage’s Secret of the Lily. Starting at level 74 players can also use Rhizomata to give a single Addersgall instantly. Collected Adersgall can be spent on the following:
- Druchole: level 45, 600 potency cure, restores 7% of Sage’s max MP.
- Kerachole: level 50, area of effect centered on the Sage, reduces damage taken by 10%, adds a 100 potency regen, 15-second duration, restores 7% of Sage’s max MP. Does not work on those under Taurochole.
- Ixochole: level 52, 400 potency area of effect heal, restores 7% of Sage’s max MP.
- Taurochole: level 62, 700 potency cure, reduces damage taken by 10%, 15-second duration, restores 7% of Sage’s max MP. Does not work on those under Kerachole.
Building Eukrasian and Addersting
Eukrasian directly changes the properties of abilities in FFXIV, including Dosis, Diagnosis, and Prognosis. For the first, it turns it into damage over time instead of instant damage. For the most part, only one Eukrasian Dosis should go on a target at a time, and then Eukrasian should be toggled off to continue direct casts of Dosis.
For Eukrasian Diagnosis and Eukrasian Prognosis the MP will skyrocket, but those spells will apply a powerful barrier to prevent incoming damage – a further 180% for Eukrasian Diagnosis and 320% for Eukrasian Prognosis. When such a barrier breaks from sustaining damage, one will gain Addersting stacks to unleash Toxikon. Eukrasian can also be altered with an additional skill:
- Pepsis: level 58, destroys current barriers in a 15 yalm radius and heals with a potency of 450 for Eukrasian Diagnosis or 350 for Eukrasian Prognosis. Allies without Eukrasian will not be healed.
Pepsis can serve as one of Sage’s best emergency healing spells, as its potency is not affected by Eukrasian’s duration or remaining protection. This makes it key to use Pepsis right before a barrier is about to end naturally. Though, this often means learning more about individual raids and the attack power of certain monsters.
